If you have submitted documents to DragonFly, please double check that you have submitted the following:
1. Electronic Signature Agreement
2. Permission to Participate
3. Drug Testing Consent Form
4. Concussion Statement
5. Provider's Physical Examination Form
6. PPE Health History
Also make sure that your forms have been approved. If any of the above is missing or approval was not received, please print the forms and bring them to the high school activities office. The forms can also be located on the school website under "Documents / Athletics & Activities / 2023-2024 Athletic Participation Packet."
Your student athlete will not be allowed to participate in practice without the above documents, including activity fee.
